- If you did not fill out the baseline survey during your enrollment session, please fill this out as soon as possible. We use this survey to continually improve FAIR. Please fill it out as though you did not yet have your FAIR account.
- Complete application and/or upload ID and address verification.
- If you are having trouble logging in to the above portal and need to submit your ID and Address Verification, please use submit your documents via Dropbox or email.
